Disease Management (DM) is coordinated care for patients with specific conditions (such as diabetes) or health risks (such as obesity). DM programmes are generally aimed at clearly defined patient populations and emphasize patient education and the advancement of self management. They are oriented to the integration of preventive and curative interventions and are based on evidence based medicine. Care is provided as much as possible in clinical pathways and is supported by a high level of modern Health Information Technology. A range of management instruments are used in DM programmes, such as feedback to professionals and benchmarking. The organizational structure is robust; goal setting and funding come from a central point.
Disease Management programmes are developing in many countries: the USA, Australia, Brazil, Japan, South Africa and also across Europe. Until now, however, European countries have not compared their DM programmes despite the fact that they function in comparable contexts: mostly with developed primary health care systems, funding from taxation or social insurances, and with a high level of influence from central government, local government, social health insurance agencies and specific state agencies. Furthermore, DM programmes in Europe are not in a steady state: they are still in the developmental phase, which makes it interesting for policy makers, professionals and researchers to learn from each other.
The conference is centred around four workshops which will take place on Thursday, January 11th. The workshop topics will be diabetes, stroke and other heart diseases, COPD, and depression and they will be facilitated by experts from various European countries (see programme). The workshops will contain medical as well as economic and managerial content. Participants are expected to bring their own expertise and experiences into the discussions.

The workshops will not solve the problems associated with every issue but should provide the basis for comparing the experiences from the perspective of various European countries.
The final day will begin with summaries of the various workshops and will conclude with a plenary discussion on the topic of setting the European Policy and Research Agenda for DM Programmes in Europe.

Based on these objectives, the ideal participants of the conference are patient representatives, experienced professionals, clinical leaders, health care managers, policy makers within insurance companies, professional unions and state agencies, researchers, consultants, lecturers and all additional persons interested in continuity of care.

Conference fees include the 2 day programme (January 11-12), congress packet, lunches and dinner on January 11.
Upon receipt of the registration form, delegates will receive a confirmation via e-mail as well as an invoice via the regular post. INIC membership fees must be paid to be eligible for discounted conference fees and conference fees must be paid in full before admission to the conference.
The deadline for conference registration is December 20th. The number of conference places is limited, if your registration can no longer be accepted, you will be notified by email.
Fees will be refunded for cancellations submitted in writing up to three weeks before the start of the conference. Cancellations thereafter will not be refunded. However, substitutions may be made at any time free of charge.
